How can you help motivate a spouse who is having trouble losing weight or finding the motivation to do so?

This spouse comes from a family of all morbidly obese people (40+ BMI’s), has been at least a little overweight/flabby his whole life, and has had a BMI of 43-47% over the past 5 years. He’s currently 6’0” and 325lbs. He hates his body and wants to lose weight/build some toned lean muscles, but can never seem to stick with a weight loss plan of his choosing and “just do it”. He always loses motivation/drive within a week or two weeks at most, and ends up giving up, eating more, and getting fatter. He has 2 children under age 5, and a wife who is very active and fitness oriented. His obesity has taken a huge toll on their marriage, particularly their sex life and her attraction to him. What have you experience and seen success with in similar scenarios?

Edit: this man’s wife eats paleo herself and cooks that way for the family, plus some whole grains like quinoa and rice for the kids. She’s cooked for him when he’s wanted to do keto and other diets, but he usually ends up eating out while at work (usually at McDonalds). He also hates workout out with his wife; he tells her she’s too advanced for him and he can’t keep up. He’s 29 years old and currently has high blood pressure, plus an extensive family history of diabetes, autoimmune diseases, high blood pressure, immobility, DVT, gout, and many other issues, so it’s not like he doesn’t know the consequences of obesity. Also, his wife is a clinical nutritionist.
